Building Community Health Together
The Greater Hamilton Health Network (GHHN) is one of the first Ontario Health Teams across the province, with a purpose of transforming healthcare in partnership with patients, families, care partners, primary care, local organizations and the community.
A collaboration of local health and social service partners the GHHN includes representation from more than 40 organizations. We are a dedicated group of professionals, organizations and people with lived experience working to co-design a patient centred, health system grounded in engagement, health equity and the local needs of the communities we serve across Hamilton, Haldimand, and Niagara Northwest.
The Greater Hamilton Health Network is supported by funding from the Government of Ontario.
WhaT is an OHT?
The Ontario government is building a connected health care system centred around patients, families and caregivers to strengthen local services, make it easier for patients to navigate the system and create seamless transitions between providers.

March 2024 GHHN Newsletter
Happy March Everyone!
I know so many of you are busy with fiscal year end activities, so appreciate you taking the time to read the newsletter. Seasons are already changing, although I am not sure we had a true winter. We have added to the team this month to look after important priorities like mental health and addictions, health equity and home care. Over the last two months, GHHN has been busy moving our acceleration priorities ahead: consulting with patients and providers about CHF and COPD for our health pathway work, completing a primary care network readiness assessment and preparing for expanding team-based care in primary care.
Supporting Young Caregivers: The Hidden Population of Caregivers
A young caregiver is a child, teen or young adult who supports a family member for a variety of reasons. It could be a parent, sibling or grandparent. They may have a chronic illness like cancer, diabetes, dementia or Multiple Sclerosis. They may have an intellectual disability like Autism or Down Syndrome. They may have a physical disability from a brain injury or workplace accident. They may have mental health challenges like depression, anxiety or PTSD. They may have addictions to drugs or alcohol. Or they may have a variety of socio-economic factors like being a new family to the country and only the child speaks English or having a single parent at home.

Building Health Equity in the Greater Hamilton Area
In the fall of 2020, the Greater Hamilton Health Network began the important work of implementing their vision to provide an equitable and seamless continuum of care that actively improves population health, while meeting the individual needs of the community.
As part of this work, the GHHN recognized and identified the need to engage the voices of communities who face systemic discrimination or marginalization to understand how this impacts access to universal and accessible health care, knowing that inequity drives poor health.
We are pleased to share our community driven Health Equity Report that shares the voices of our communities and our bold vision for an equitable and culturally safe healthcare system that is patient centered.
