Building Community Health Together
The Greater Hamilton Health Network (GHHN) is one of the first Ontario Health Teams across the province, with a purpose of transforming healthcare in partnership with patients, families, care partners, primary care, local organizations and the community.
A collaboration of local health and social service partners the GHHN includes representation from more than 40 organizations. We are a dedicated group of professionals, organizations and people with lived experience working to co-design a patient centred, health system grounded in engagement, health equity and the local needs of the communities we serve across Hamilton, Haldimand, and Niagara Northwest.
The Greater Hamilton Health Network is supported by funding from the Government of Ontario.
WhaT is an OHT?
The Ontario government is building a connected health care system centred around patients, families and caregivers to strengthen local services, make it easier for patients to navigate the system and create seamless transitions between providers.

GHHN October 2023 Newsletter
It continues to be an exciting time for the GHHN! On September 27th, it was announced that the GHHN is one of 12 OHTs chosen to implement an advanced workplan and work towards OHT designation at a rapid rate. What does this mean? It means we are working towards being responsible for the full funding envelope for our community and eventually taking on the responsibilities of home care. This is a massive announcement and has a
lot of implications for our work moving forward. Over the month of October, we expect to hear more about specific deliverables around primary care, pathways and system navigation.

Happy Summer Partners! The first six months of the year have already passed us by. It has been filled with hard work, complex issues to discuss and great successes. Over the past 6 months we have brought on amazing initiatives like the lower limb preservation project, LEGHO community program and completed 2 more women’s health days. None of this would be possible without partnerships, relationships, and dedication to serve patients. We see many leadership changes across the GHHN – we wish everyone transitioning the best of luck in their new roles, happy retirement to some and a warm welcome to new leaders and board members.
The Importance of Gender Affirming Care
Bodily autonomy continues to face unprecedented challenges, even in the 21st century. Globally, gender affirming care has been politicized to justify restricting or limiting access to – for many – life saving care. As anti-trans groups continue to organize locally and globally, we stand in support of our local Two-Spirit, transgender, non-binary, and gender diverse communities. The Greater Hamilton Health Network (GHHN) recognizes the right to receive gender-affirming care for all in our catchment area and beyond.

Building Health Equity in the Greater Hamilton Area
In the fall of 2020, the Greater Hamilton Health Network began the important work of implementing their vision to provide an equitable and seamless continuum of care that actively improves population health, while meeting the individual needs of the community.
As part of this work, the GHHN recognized and identified the need to engage the voices of communities who face systemic discrimination or marginalization to understand how this impacts access to universal and accessible health care, knowing that inequity drives poor health.
We are pleased to share our community driven Health Equity Report that shares the voices of our communities and our bold vision for an equitable and culturally safe healthcare system that is patient centered.
