As part of system transformation currently underway in Ontario’s healthcare system, the Greater Hamilton Health Network (GHHN) OHT is seeking a Family Medicine/Primary Care Clinical Leader to provide clinical expertise and operational leadership for the activities and planning of Primary Care Centered Integrated Care Pathways.

At the core of this work is the development of “shared understandings” between first contact/primary care and the secondary care/specialized services required to support patients and their families along specific health care journeys related to prioritized clinical conditions.  In this context, a shared clinical agreement describes the optimal use of primary care and consultative services to diagnose, manage and support priority clinical conditions including medical workups, access to relevant diagnostic services, appropriate referrals, shared care protocols and support for hand offs back to primary care.  These agreements hold the fundamental premise that the best outcomes for patients and the system occur when the central elements of primary care: continuity, coordination, comprehensiveness, and access are core to the design of each pathway.

The central health journey is one shared by the patient and their family with their family physician or primary care provider and the primary care team.  Primary care centred pathways also often involve consultation or referral to relevant community agencies and programs.  The involvement of these key partners is also predicated on centered the core elements of primary care as outlined above.

The GHHN Primary Care Integrated Health Pathway lead will work effectively and as needed with Greater Hamilton Health staff, primary care network members, relevant specialist colleagues, community agencies, patient advisors, and stakeholders relevant to each priority condition.  Each pathway is expected to first ensure clinical consensus for a sustainable population health approach for the medical care of patients presenting with/experiencing the priority condition, oversee the translation of that co-design into a relevant pathway  and then, with the support of the GHNN, implement the knowledge translation and implementation of that pathways into the clinical and community stakeholder group responsible for the care of that condition.

All pathways are expected to include patient involvement and sign off to ensure lived experience and the patient perspective are included in each step.

Key Competencies:

  • System thinker with understanding of clinical leadership in health system collaborations.
  • Trustworthy and credible clinician, able to demonstrate the ability to reflect and integrate the diverse viewpoints of different system stakeholders, including specialists from relevant disciplines
  • Able to include the voice and experience of patient/client and caregiver partners to ensure design of each pathway optimizes their experience and ability to participate in their own care
  • Excellent communicator, strategic thinker, and team player with a passion for health system improvement and the ability to influence change.
  • Self-directed and highly motivated; able to handle and navigate clinical and operational ambiguity with multiple stakeholders.
  • Proven ability to build, strengthen, and maintain relationships and partnerships with stakeholders from diverse communities, backgrounds, and experience.
  • Expertise, understanding, and/or passion for a population health approach and population health management.

Qualifications and Experience:

  • A practicing Family Physician holding a valid certificate of registration and in good standing with the College of Physicians and Surgeons of Ontario.
  • Actively practicing within Greater Hamilton and Area.
  • Expertise, understanding, and/or passion for population health management, quality and process improvement and digital health.
  • Ability to manage competing demands and meet tight timelines.
  • Knowledge of provincial health care structures, processes, and funding mechanisms is strongly preferred.
  • Excellent communicator, strategic thinker, and team player with a passion for health system improvement and the ability to influence change.
  • Experience in applying a health equity lens to projects/initiatives aligned with the quadruple aim (population health, patient experience, per capita cost, provider experience) preferred.
  • Formal leadership training, and experience in academic detailing or mentoring considered an asset.

Compensation: $165/hour

Predicted Time Commitment: estimated to be between 10-12 hours per month.

About Medical Assistance in Dying (MAiD)

MAiD is supported through the following changes: “In February 2015, the Supreme Court of Canada ruled in Carter v. Canada that parts of the Criminal Code would need to change to satisfy the Canadian Charter of Rights and Freedoms. The parts that prohibited medical assistance in dying would no longer be valid. The Supreme Court gave the government until June 6, 2016, to create a new law.

Duties and Responsibilities

  • Answering phone calls, providing support to patients and caregivers who may have questions or wish to self-refer.
  • Accept referrals, triage patients and liaise with team assessors/providers.
  • Maintain a regular caseload, estimated 12-15 open cases at any given time.
  • Provide supportive health teaching for patients and caregivers relating to MAiD eligibility criteria and processes.
  • Coordinate, attend and participate in patient assessments with team a team including physicians and/or Nurse practitioners.
  • Coordinate and assist with all aspects of provision (i.e. organize IV starts, drawing up of meds prn, preparing patient and family etc.)
  • Support patients with resources should they wish to donate their organs.
  • Maintain accurate documentation (SOAP notes, MOH forms, outcome tracking etc.),
  • Foster and maintain collaborative relationships with Primary Care/referring MRP.
  • Liaise with community partners (e.g. HCCSS, Visiting nurses) as needed.
  • Provide back-up coordination coverage for team members as needed.
  • Participate in quality and performance improvement activities
  • Participate in community tables and planning committees
  • Participate in sustainability planning for the team, considering innovative options to ensure long-term viability of the team.

Education and Experience

  • Registered Nurse or Registered Practical Nurse in good standing with the College of Nurses of Ontario with experience and expertise in intravenous starts.
  • Or Registered Social Worker with experience supporting patients through MAiD and strong knowledge of a palliative approach to care.
  • Experience providing a empathetic approach to care and supporting both patients and their caregivers.
  • 3-5 years of clinical experience preferred

Specialized skills and knowledge:

  • Strong understanding of all MAiD legislation and processes
  • Strong knowledge of relevant legislation including but not limited to Freedom of information and Protection of Privacy Act, Regulated Health Provisions Act.
  • Exceptional and compassionate interpersonal and communication skills

Location(s)
Office location may vary across Hamilton FHT hub sites.  Travel is a requirement of the role.
