The Greater Hamilton Health Network is helping connect people to services and resources in Hamilton, Haldimand and Niagara Northwest. Many of these services are available 24/7, please read the descriptions below and visit their websites for more details.

It can sometimes be difficult to know where to go for health care, social services (including mental health and addictions services) and other resources available in our community. That’s why we’ve developed this resource to help connect you to the services you need.

We respect and appreciate that there are many words to describe those who access and receive health care services. Please note that for the purposes of this webpage we will use the word patient.

Patient Online Appointment Booking

A 24/7 secure online booking platform for patients and their caregivers in Hamilton, Haldimand and Niagara Northwest to request or directly schedule appointments with their primary care clinician using a desktop or mobile device.

211 Ontario

2-1-1 Ontario is a free 24/7 confidential helpline (dial 2-1-1) and searchable online database for all community, social, health and government services.
Bilingual and translation are available.

Health811

Health811 is a free, secure and confidential service Ontarians can access 24 hours a day, seven days a week to receive health advice from a registered nurse by calling 811 (TTY: 1-866-797-0007), chatting online or navigating to Ontario.ca/health811.

988 National Suicide Crisis Helpline

Suicide prevention support for people who need it, when they need it most. 9-8-8, Canada’s new three-digit suicide crisis helpline, is now available to call or text, in English and French, 24 hours a day and seven days a week, across Canada.

COAST Hamilton

The Crisis Outreach and Support Team (COAST) is a partnership between Mental Health Workers at St. Joseph’s Healthcare and specially-trained officers of the Hamilton Police Service. The program serves the residents of the City of Hamilton who have serious mental health issues and are in crisis. Crisis line available 24/7.

CAST Haldimand

24-hour mental health crisis service for ages 16+ provides crisis assessment, short-term crisis counselling, and referral to appropriate community services as required.

Haldimand Norfolk Reach

A crisis intervention service, 24 hours a day, 7 days a week for children, youth (under age 18) and their families. A qualified professional is available to talk with you by phone any time of the day or night about what is happening.

COAST Niagara

The Niagara Regional Police Service Crisis Outreach and Support Team (COAST) provides outreach and support to Niagara residents who are in crisis with mental health issues. Crisis line is available 24/7.

Kids Help Phone

Kids Help Phone’s e-mental health services are available 24/7 across Canada.  Providing free, multilingual and confidential support for kids, teens and young adults from coast to coast to coast.

ConnexOntario is a free 24/7 confidential navigation and information service to access Ministry of Health-funded mental health, addictions and problem gambling services.

Ontario Structured Psychotherapy West

Offers free, short-term, evidence-based cognitive behavioural therapy (CBT) for adults experiencing mild to moderate depression, anxiety, and anxiety-related concerns.

Digital Access to Patient Records

View your personal health records online through a digital health portal, secure web-based platform.

ConnectMyHealth

ConnectMyHealth is a free patient portal that allows you to view your health information from Ontario Health West Region healthcare organizations in one, consolidated view.

The Greater Hamilton Health Network, Medical Assistance in Dying Steering Committee, was established to develop a region wide approach across Hamilton, Haldimand, and Niagara Northwest for the delivery of MAiD services to meet the needs of patients seeking MAiD services. Urgent Care Ontario

Whenever possible your family doctor is your first point of access for care. A virtual urgent care appointment may be right for you if you or a loved one have a medical issue that is NOT life-threatening, but requires urgent medical attention.
