HealthPathways- New FREE resource for Primary Care Providers

Community HealthPathways

The Greater Hamilton Health Network, Burlington Ontario Health Team and Middlesex London Ontario Health Team are thrilled to announce the launch of HealthPathways in Canada. The agreement marks a milestone for integrated health systems by introducing a proven model that enhances health outcomes through a collaborative approach with our local healthcare providers.

What are Community HealthPathways?

Community HealthPathways provides primary care physicians and clinicians with access to a suite of locally agreed-upon pathways covering assessment, management, and referral to community and hospital care services. HealthPathways began its journey in Canterbury and has since been adopted across New Zealand, Australia, and the UK. We will be the first region in Canada to implement Community HealthPathways, supporting healthcare professionals by providing locally relevant, evidence-based guidelines and resources.

Why are Community HealthPathways Needed?

HealthPathways addresses critical healthcare challenges such as:

    • Reducing variations in patient outcomes
    • Minimizing unnecessary specialist referrals
    • Streamlining administrative inefficiencies

The Greater Hamilton Health Network (GHHN) HealthPathways platform will offer:

    • Integrated Care Pathways
      Host pathways for assessing, managing, and referring chronic diseases and other conditions, offering evidence-based guidance, localized care directories, referral information, patient education resources, and more.
    • Real-Time Access
      Provide real-time access to a platform for primary care providers and healthcare clinicians to easily access best practices, local options, and patient resources.
    • Efficient Referrals & Education
      Offer guidance for accurate referrals, plus electronic and printable materials for patient education.
    • Access to Best Practices
      Provide access to 600+ care pathways from other regions, delivering best practice guidance at the point of care.
    • Collaboration Across Specialties
      Enhance coordination and provide more care options through cross-specialty collaboration.
