Primary Care Network

About Us

The Hamilton Family Medicine (HFAM) has evolved into the Greater Hamilton Health Network Primary Care Network. We would like to offer special thanks to everyone who contributed to the development of and participated in HFAM and look forward to this next chapter as the GHHN Primary Care Network!

The Greater Hamilton Health Network Primary Care Network (PCN) is your one-stop shop for primary care in our area.

PCN aims to improve the delivery and coordination of care for patients by connecting and supporting primary care providers and the primary care community so that we may thrive together within our local Ontario Health, the Greater Hamilton Health Network (GHHN). You can learn more about the GHHN here.

PCN is a collection of primary care providers, workers, and organizations that provide structure to primary care in the GHHN to ensure the voices of all primary care.

Primary Care Network (PCN) Functions

In alignment with Ontario Health, the GHHN PCN has 5 functions:

Amplify the primary care ‘voice’ and strengthen its influence with healthy system planning, including within the GHHN.

Understand the needs and priorities of our patient populations and the local primary care sector.

Support local and provincial priorities in primary health and community care planning.

Optimize resources equitably and guide new investments in primary care.

Enhance the primary care practice environment.

Guiding Principles

To underpin the PCN’s functions, it adheres to four foundational guidelines that inform the structure, strategy, and operations of the PCN:

Patient Care

We are focused on improving the delivery, coordination, integration and collaboration of high-quality primary care for all within the GHHN catchment area.

Equity

We embrace a health equity lens in everything we do, with a focus on the needs of historical, persistenly and/or systematically marginalized populations.

Impact

We work to make meaningful change for our patients, communities, and ourselves by taking action, advocating, collaborating, and trying new things. In this regard, the PCN is a vehicle for advocating and securing primary care investments.

Sustainability

We deliberately structure our network to have long term viability of primary care.
