Why does the GHHN Primary Care Network exist?
Our PCN aims to enhance patient care and provider experience by connecting and supporting the primary care community within the GHHN. This network ensures ALL primary care voices are not only heard but influence decision making and priority setting.
Who can join the GHHN Primary Care Network?
Our PCN is open to:
- All family physicians, regardless of practice model or organizational affiliation.
- All primary care clinicians, including interdisciplinary healthcare professionals.
- All primary care learners.
- All administrators, leaders and other non-clinical workers in primary care.
- Anyone who champions primary care and aligns with the purpose of the PCN, such as public health, educators, patients, advisors and more!
Does the GHHN Primary Care Network have a membership fee?
Joining the PCN – and receiving all it’s benefits – is currently free!

What is the Greater Hamilton Health Network?
The Greater Hamilton Health Network (GHHN) is one of Ontario’s first Health Teams, focused on transforming local healthcare through collaboration with patients, providers, and community partners. Our goal is to improve patient outcomes and experiences by building an integrated care system guided by frontline expertise. The GHHN and its Primary Care Network work together to share tools, supports, and infrastructure to achieve these shared goals.
