PCN Structure

Council and working group

The PCN has developed a robust governance structure in alignment with the GHHN and Ontario Health guidelines. This structure includes the Primary Care Network Leadership Council (PCNLC), the decision-making body for the PCN, and operational committees, which can be established and dissolved as needed based on PCN priorities.

The PCNLC reports directly to the GHHN Board of Directors, with the PCNLC subcommittees reporting into the PCNLC (the Digital Committee has dual reporting to the PCNLC and the GHHN Digital Health Secretariat).

Including interdisciplinary healthcare professionals.

All Primary Care Learners

Including residents, medical students, and all other healthcare learners. 

All Administrator, Leaders

or other non-clinical workers in primary care.

Anyone Else Who

champions primary care and aligns with the purpose of our PCN, such as public
health, educators, patient advisors and more!

What is the GHHN?

The Greater Hamilton Health Network (GHHN) is one of 54 Ontario Health Teams across the province with a purpose of transforming healthcare in partnership with patients, families, care partners, primary care, local organizations and the community. How is the PCN connected to the GHHN?

The PCN is the local Primary Care Network for the catchment area of the GHHN, including Hamilton, Haldimand, and Niagara Northwest.

While the PCN reports into the GHHN Board of Directors, the PCN is responsible for setting our strategic direction, managing primary care-related funds flowed through the GHHN, and communicating to and advocating on behalf of the PCN. The GHHN and PCN work together to share in-kind tools, supports, and infrastructure to achieve goals.
